FINANCIAL & INSURANCE POLICY
During your consultation and before surgical care Dr. Ehsan's
financial staff will inform you of the fees. The fees reflect
the degree of difficulty and risks involved in the procedure.
We believe that understanding the fees before surgical therapy
is important for patients.
Please bring all medical and dental insurance information (if
applicable) so our staff can review your coverage with you. Our
ultimate goal is to offer outstanding surgical care at the most
reasonable cost to our patients. It is very important for patients to
understand that your insurance policy is an agreement between you and
your insurance carrier. Our staff will be happy to assist you in
completing the necessary forms. Our intent is to maximize insurance
payments thereby making your out-of-pocket costs low.
Following your initial co-payment and surgery, we will process
your insurance forms for you and will notify you of your reimbursement.
Most insurance companies will respond within four weeks sending
our office and you notification. Please review your insurance
carrier's letter as they will explain your reimbursement/payment.
Any remaining balance for surgical services rendered after the
insurance has paid its portion is the responsibility of the
patient. We cannot exactly predict what insurance benefits individual
insurers will pay. This depends upon your deductibles, coverage
limits, contract exclusions and carrier fee schedules. To give
you the best possible estimate, we will verify your coverage
with your insurance carrier. The information they provide us
is usually but not always accurate. If for any reason the insurance
company denies payment or payment has not been received after
two (2) months for the services rendered, you will be responsible
for the entire amount.
We are a provider for most of the major dental and medical insurance plans to minimize your out-of-pocket costs.
For your convenience we accept credit card (Visa, Mastercard,
American Express), and cash. We also have access to Dental Fee
Plan by CapitalOne which is a lending company that can do payment
plans for certain patients. Dental Fee Plan by CapitalOne offers
a line of credit for health/dental care needs. They provide
two options: 3 or 6 month interest free option (amounts $300-$25,000)
or 18 to 60 month plan with fixed interest rates (amounts $1000-$25,000).
These plans offer no initial payment, fixed rates, and no prepayment
penalty.
We can also make arrangements for a convenient payment plan, but this must be set up prior to the actual procedure.
